At the Korean Hospital Pharmacists Association spring academic conference held at COEX Convention Center on the 20th, Professor Moon Jeong‑yeon of the Keck Graduate Institute School of Pharmacy presented on the importance of medication reconciliation during transitions of care.
She argued that pharmacist‑led medication reconciliation can reduce medication errors that arise during patient admission, discharge and ward transfers, and called for formal institutionalization of the practice in Korea.
The presentation also reviewed medication reconciliation models from major countries, illustrating how they are implemented abroad.
